We are delighted to introduce United in Change – Avenida House, a newly licensed residential care facility nestled in the heart of Moreno Valley, CA. With a capacity for up to 4 residents, our facility is designed to provide a warm, supportive environment for adults aged 18-59.
At United in Change – Avenida House, we pride ourselves on our friendly and knowledgeable staff who are dedicated to ensuring the well-being and comfort of each resident. We are pleased to be a pet-friendly community, understanding the positive impact that companionship can have on our residents’ lives.
Our Care Team is committed to creating a vibrant community where residents can connect with friends and family through a variety of engaging activities: Sunday Brunch, Game Night, Story Sharing Groups, Music and Dance Performances, we offer a range of events that foster camaraderie and enjoyment. Additionally, we provide practical support such as Individualized Meal Plans, Medication Management, Assistance with Activities of Daily Living, and Transportation services.
For added peace of mind, our awake overnight staff ensures round-the-clock safety and support, our Care Team is available 24/7 to address any needs that may arise. Our goal is to promote independence and uphold the dignity of each resident, respecting their individuality and right to self-determination.
